Kameika Lee, NP, doing business as Rooted Women’s Health, is the Health Information Custodian under PHIPA. This means we are legally responsible for the personal health information collected, used, and disclosed in providing your care.

  • Health card and insurance details
  • Medical history, current concerns, and symptoms
  • Test results, treatment records, and clinical notes

  • Communication records with our practice

We use Jane App, a PHIPA-compliant Canadian electronic medical record system, to store your information securely. All data is encrypted in transit and at rest. Access is restricted to authorized personnel only.

7. Your Rights Under PHIPA

You have the right to:

  • Access your personal health records
  • Request corrections to inaccurate information
  • Withdraw consent for the use or disclosure of your information (subject to legal requirements)
  • File a complaint with our Privacy Officer or the Information and Privacy Commissioner of Ontario
